Quality Improvement Proposal

Slide 1

Hello! My name is ____. I am introducing the quality improvement project that was chosen as a strategic focus of the behavioral health department of Albany Medical Center.

Slide 2

  • Executive Summary

One of the most critical areas that will have high-level quality improvement to maintain the Magnet status and improve patient outcomes has been identified as the behavioral health department of Albany Medical Center. The strategic plan will focus on such priorities as transformational leadership, structural empowerment, and exemplary professional practice (Kleine et al., 2024). In order to maintain Magnet accreditation, the department aims to increase the national presence even more, improve well-being among the staff, and improve the standards of the care offered to the patients. The Lewin change theory would therefore come in handy in the change management process, including the incorporation of all the stakeholders (Endrejat & Burnes, 2022). The SWOT analysis is conducted to evaluate the strengths, weaknesses, opportunities, and threats (Puyt et al., 2023). The fresh policy on quality improvement will be included in the program, and more recent approaches to the training and development of the staff. The old practices will be stored in case the departmental practices are streamlined to the overall objectives and standards of the hospital. The enhancement of internal processes will be done with the help of nursing simulation centers, and ongoing learning and development will be stimulated by nursing scholarship and evidence-based practice (King et al., 2021). The organizational policies are well synchronized with the strategic objectives of the department so that there is a singular way of enhancing patient outcomes, performance of the staff, as well as making the best use of resources.

  • Quality Improvement Proposal

The quality improvement proposal of Albany Medical Center behavioral health department is a patient waiting time decrease to promote an efficient scheduling process as one of the strategic priorities to improve. The enhancement will be aimed at improving access to behavioral health services using technology-based solutions, including telehealth services and state-of-the-art scheduling software. The department aims to minimize waiting times in appointments in order to improve patient trust and satisfaction and deal with the growing demand in mental care (Hospital Safety Grade, n.d.). The proposal emphasizes collaboration between mental health practitioners and nurses and information technology (IT) specialists to promote the seamless integration of such technologies (Isidori et al., 2022). The program helps the department meet its strategic goal of offering economical, prompt and equal behavioral health services based on the mission which focuses on enhancing the health outcome of the community by Albany Medical center.

Slide 4

  • The Rationale for Establishing a Strategic Priority

The reasoning behind the strategic direction of decreasing patient wait times within the behavioral health department is that there is growing demand of timely mental health services. The wait times may lead to worsened mental health problems, then the patients may cease to be engaged in care, which has more significant impacts in underserved groups (Sanderson et al., 2020). This would be the priority of the mission of the department, which is to enhance community access to equitable cost-effective behavioral health services and health outcomes overall (Hospital Safety Grade, n.d.). The overall goal of the proposed quality improvement initiative is to make the scheduling process more streamlined and to start telehealth services to reduce wait times, thereby increasing and enhancing care delivery to behavioral health patients. Its emphasis is on two aspects: the streamlining of patient appointments through technology (telehealth platforms and advanced scheduling systems) and interprofessional collaboration between the staff to make sure that such systems are implemented properly.

The literature on evidence-based sources is critical on the fact that there is a dire need to have timely access to behavioral health services. Tele-based solutions have been shown to offer patients in remote or poorly served areas additional access to services and improve their use (Butzner & Cuffee, 2021). The proposal matches findings proving that innovative service delivery models can help bridge the gap in mental health care while being consistent with Albany Medical Center’s and Department goals.

  • Implications and Consequences

The mentioned priority to minimize wait times of patients in the behavioral health department has a range of logical implications and consequences that have a substantial influence on the delivery of care. On the bright side, the priority can improve patient access to prompt mental health care, minimize the worsening of undiagnosed conditions, and overall improve patient satisfaction and trust (Sanderson et al., 2020). Less waiting time also leads to higher patient outcomes since early intervention and follow-up are the most important factors in addressing mental health issues accordingly. Moreover, telehealth use and effective time management systems will help gain maximum out of the available resources and increase staff efficiency, which will also guarantee the financial sustainability of the department (Butzner and Cuffee, 2021). Such practices would also be expensive to implement and much of the staff would have to be trained on the new technology and this could, in some cases, lead to resistance by the staff and other patients. The implementation is prone to experience delayed implementation and staff and patient dissatisfaction without a proper and timely solution (Robinson et al., 2020). But with a successful execution, the wait time reduction priority will create a more responsive, fair and patient centered behavioral health service that aligns with the strategic objectives of Albany Medical Center.

  • SWOT Analysis

Strengths

Albany Medical Center has exhibited a number of strengths resonating with strategic priorities to enhance patient satisfaction and expand access to care. The institution boasts of a well-developed infrastructure to implement telehealth services, with strong electronic health records (EHR) and convenient person-scheduling systems (Hospital Safety Grade, n.d.). In addition, the institution has a highly trained staff that is equipped with behavioral health expertise that makes it self-assuring and provides quality care. Close adherence to interprofessional collaboration among healthcare providers also increases the capacity of the healthcare center to provide integrated and patient-centered care that would lead to ultimate success in terms of achieving the strategic objectives.

Weaknesses

The Albany Medical Center has a number of weaknesses that may hamper the successful implementation of the telehealth programs. There is not enough funding, which will not allow investing in new telehealth technologies and scheduling applications that will slow progress (Makhni et al., 2020). The resistance to change because of the additional training that the staff will have to go through will be a hindrance to the process of adoption and integration. The fact that the center relies on IT systems which need to be upgraded may be the cause of technical difficulties, thus making the process of implementation even more complex ( Hospital Safety Grade, n.d.). Secondly, under staffing in the rollout can overstretch the existing services, which can impact patient care and satisfaction.

Opportunities

Albany Medical Center can also take advantage of certain opportunities to consolidate the services and expand coverage, such as the increasing population need for accessible and timely behavioral health services in the post-pandemic period, which presents the possibility of meeting a crucial need. Healthcare reform policies, government incentives and investments in the development of telehealth provide extra funding to continue the development of services in the field (Werf et al., 2021). Hospital can also expect the introduction of new technologies to ease the scheduling process, making it even lighter and more efficient ( Hospital Safety Grade, n.d.). By offering timely, patient-centered behavioral services, Albany Medical Center will be able to enhance its image, which will even boost the number of patients and competitiveness within the healthcare market segment.

Threats

Albany Medical Center has a number of issues that might impede the success of telehealth programs. Strict policies on reimbursement of telehealth and limited licensing that can impede growth are seen as a political and regulatory barrier (Makhni et al., 2020). Also, other behaviorally-advanced healthcare facilities vie in the market. Both elevated levels of utilization of telehealth and scheduling technologies provide the opportunity for data breaches or cybersecurity assaults, which would jeopardize patient trust (Makhni et al., 2020). The uncertain economic situations can also restrain quality improvement activities based on the funds available, which presents additional obstacles to the work on strengthening care delivery and infrastructure.

  • Political Landscape and Strategic Priorities

Within the Albany Medical Center political environment and strategic goals, the process of re-engineering patient wait times in the behavioral health department fits as part of the community health enhancement objectives of the hospital and the overall policies of healthcare to provide more affordable care. Funding, and policy reforms based on political support of mental health services, particularly following heightened demand in the COVID-19 pandemic, have led to the promotion of telehealth and integrated care models (Goldman et al., 2020). The desire of the Albany Medical Center to increase service provision, patient satisfaction, and health outcomes also demonstrates the organization is in line with state and federal health policies, which aim at ensuring mental health is made accessible (Hospital Safety Grade, n.d.). However, the political environment has some opposing effects, such as remodeling telehealth reimbursement regulations, government policies, and access to scarce resources (Goldman et al., 2020). These are the political variables that make a great factor towards the assurance of industry standards and regulations.

Slide 6

  • Key Performance Indicators

To gauge the level of success in the quality improvement initiative in the behavioral health department, the key performance indicators (KPIs) need to be linked to the objectives outlined in the balanced scorecard of the department. The most pertinent KPI on the basis of financial revenue growth illustrates the drawal of additional patients towards health services and economical care delivery (Donnell et al., 2020). Telehealth services will be implemented using revenue increase as the measure. The increase in revenue will be 12 percent in the coming year. Another highly valuable KPI in the customer sphere is patient satisfaction (Gavurova et al., 2021). When patient satisfaction scores reach 90% when the service is provided through telehealth to offer behavioral health services, then the KPI will be assessed. The measures are in direct relation to the fact that the department aims to use the power of technology to improve the care delivery process and make sure that patients are pleased with the care experience.

One of the indicators within the internal process area that would be essential to the initiative would be patient wait time. As the initiative would be focused on reducing the wait time from one week to one day due to the enhancement of scheduling and the use of telehealth, the objective of more timely care would be achieved by assessing the mean waiting time between appointments and care delivery. The second highly relevant KPI in the learning and growth area is the rate of staff turnover, which indicates whether the initiative to build staff expertise and ongoing training is effective. Reducing the turnover by 15% can be seen as a target, which indicates the problem of the supporting work environment that is powered by efforts like a cultural competency training program. The KPIs will provide highly useful information about the efficiency of the quality improvement program and, thus, all strategic priorities will be achieved, such as better patient care, financial stability, and a highly qualified and satisfied workforce.

Slide 7

  • Stakeholders Identification

The feedback of stakeholders is a significant feature of any quality improvement project as it can be used to gather valuable information related to the effectiveness of the offered changes and avert the situation when the needs and the concerns of all interested parties will be overlooked. The involvement of the stakeholders in the process will create a feeling of ownership, engagement, and alignment with the objectives of the initiative, and it is more likely to be successfully implemented (Loureiro et al., 2020). Stakeholder collaboration entails determining the stakeholders, collecting inputs, whether in the form of a survey, interview, or focus group, and constant communication during the project period (Loureiro et al., 2020). This partnership can aid in streamlining the initiative, troubleshooting, and adjusting to altered conditions in a manner that integrates efforts on quality improvement to organizational and community requirements.

The most important stakeholders in the quality improvement initiative of the behavioral health department will be the healthcare providers (physicians, nurses, mental health professionals), the administrative personnel, the patients, and the families. Stakeholder support will be important as healthcare providers and staff will participate directly in the implementation and modification of the telehealth solutions proposed and care coordination processes (Loureiro et al., 2020). Patients are essential stakeholders because the kind of feedback they will provide about new telehealth and care delivery practices will indicate whether the quality improvement initiative would be productive in the long-term (Loureiro et al., 2020). The family and care providers can be relevant stakeholders in issues relating to behavioral health and provide more data on patient satisfaction and care adherence. The stakeholders will be approached with feedback, as all groups will be involved by providing input and then basing program improvements that will contribute to enhancing patient outcomes (Loureiro et al., 2020). The intended improvements in terms of quality of care and patient satisfaction require an iterative feedback loop to be achieved.

  • Value of Stakeholders’ Feedback and Collaboration Strategy

The collaborative strategy among stakeholders is attained by clearly established channels of communication, openness, and active involvement in all stages of the quality improvement journey. Stakeholders should be involved in every aspect of quality improvement through meetings, surveys, and feedback sessions performed regularly (Haesebaert et al., 2020). In line with this idea, therefore, the objectives of the initiative have to coincide with the interests and priorities of the stakeholders to create a common vision of success. Being able to inform the stakeholders of progress and results while providing a mechanism to incorporate some feedback from the stakeholders in some of the decision-making processes enhances the sense of culture in collaboration (Loureiro et al., 2020). Stakeholder support is valuable in terms of the impact on the success of a project. The additional involvement of stakeholders would enhance the chances of implementation, sustainable change and improved patient outcomes. The stakeholders will be in a better position to promote the initiative towards enhanced care delivery.

Slide 8

  • Role of Change Theory

One model that can inform the changes necessary to implement a quality improvement initiative is Lewin change theory, which can be broken down into three stages: unfreezing, changing, and refreezing. The unfreezing stage focuses on preparing the behavioral health department to change by creating awareness of the necessity to change, e.g., waiting times of patients in the queue or care coordination (Endrejat and Burnes, 2022). The unfreezing process concerns resistance to change, and the aim of the process is to overcome issues by addressing the concerns and establishing a shared understanding of the necessity to improve patient care and organizational performance (Endrejat and Burnes, 2022). The unfreezing stage is critical with leadership and frontline staff buy-in to create a commitment to the suggested quality improvements.

After the unfreezing stage is accomplished, the changing phase takes place. New practices, processes and technologies are introduced as part of the change step. The use of telehealth systems to provide remote medical care and train on cultural competency could be one such instance in the case of the behavioral health department of Albany Medical Center (Endrejat & Burnes, 2022). Good communication and follow through is also of much essence during the change phase as the employees experience resistance in an attempt to change to their new procedures. Lastly, the refreezing step will help to stabilize the changes in the culture and practices of the department. The refreezing process involves making sure that new practices are sustained through continuous training, performance, and feedback loops (Endrejat & Burnes, 2022). The model by Lewin is sustainable as the changes are integrated into the daily activities in the department, and quality improvement becomes a new standard of care.

  • Interpretation

The change theory by Lewin is rather strong in providing a system to be used when managing change within the healthcare system. The theory advocates the application of a systematic approach in transforming organizational behavior. Lewin model is applicable in ensuring change when it comes to employing a quality improvement project, like the improvement of the Albany Medical Center behavioral health department (Endrejat and Burnes, 2022). The aspects of the unfreezing stage, such as creating urgency, promoting open communication, and surmounting resistance, are considered best practices in change leadership. Leaders can reduce skepticism and promote collaboration by involving the stakeholders at an early stage and providing them with a rationale as to why the changes are necessary. During the changing stage, there has been effective leadership that guided, supported, and provided resources to ensure that the staff adopts new technologies or processes, e.g., telehealth. Finally, the refreezing process establishes itself through strengthening and incorporating the change through ongoing feedback and performance reviews (Endrejat and Burnes, 2022). During the refreezing phase, the change becomes integrated in the organizational culture and the employees proceed with the change. Utilisation of the Lewin theory offers a systematic and adaptable approach to developing quality improvements in care which are meaningful and sustainable.

Slide 9

  • Policy Recommendation

To implement the proposed quality improvement initiative on behavioral health services improvement at Albany Medical Center, a new policy to integrate telehealth services into the care model of the department should be developed. The policy will assist in defining the rules on the utilization of telehealth technologies to ensure accessibility, security, and quality of remote behavioral healthcare. The policy should indicate criteria for the selection of the technology platforms so that the platforms selected are Health Insurance Portability and Accountability Act (HIPAA) compliant and patient-friendly as well as provider-friendly (Kels, 2020). The policy must also outline the guidelines for how remote assessments, therapy sessions, and follow-up care will be conducted in a manner that will deal with the logistical challenges of patient access and equity. Such a policy change would mean that telehealth services would be properly incorporated into the overall workflow to achieve shorter wait times among patients and better care delivery.

In addition to a new policy in telehealth, the current policies on training and development of the staff should also be checked and updated to accommodate a mandatory program to train them on cultural competency. The updated policy will respond to the growing necessity of providing health professionals with an understanding of the cultural beliefs of patients that should be respected, particularly in a multicultural community. They should be trained in areas of enhancing communication skills, cultural barriers to care, and delivering equal treatment to patients irrespective of their background (Kels, 2020). The hospital will offer an inclusive patient-centered care environment by making sure that all staff members, including mental health professionals, are trained. The departmental strategic priorities of encouraging interprofessional collaboration and raising patient satisfaction will be in line with the policy change.

Slide 10

  • Policy Implications and Justification

The policy changes to include telehealth services and provide mandatory cultural competency training would result in implications for the enhancement of care quality in Albany Medical Center’s behavioral health department. Introducing telehealth to routine care involves a broader shift towards digital healthcare solutions that offer more access to care, particularly for underserved populations. This would also be of assistance to the policy since there would be a reduction in waiting time, hence enhancing a sense of overall patient satisfaction. It is necessary to introduce a cultural competency training policy to deal with the various needs of patients (Liu et al., 2020). Such policies are justified according to the quickly rising need for accessible and patient-centered care with the growing concept of cultural competency that is a leading factor in enhancing patient outcomes through trust-building and minimizing health disparities (Liu et al., 2020). The implementation would directly facilitate and complement the existing strategic priorities on achieving better staff efficiency and patient care as well as organizational performance.

Slide 11

  • Conclusion

To sum up, the strategic priorities and quality improvement strategies of the behavioral health department at Albany Medical Center will positively impact the delivery of care to patients, efficiency of operations, and customer satisfaction. The policies that facilitate the allocation of resources and cultural competency training also contribute to achieving the above goals, thus satisfying the needs of the department both internally and externally. The use of the Lewin theory and other theories of change will ensure planned changes are effectively managed. The success will be assured when there is support from stakeholders. Such a department would develop significant differences in patient outcome improvement and become a source of such improvements in the hospital environment with the aim of reinforcement via relevant policy recommendations.
